our Admissions Department at (508) 879-8000 or fill
in the request for information on-line and we will
contact you so that we may assist in answering any
of your questions. We also will schedule an appointment
for you to tour our facility. If it is determined
that placement at St. Patrick's Manor is appropriate
for your loved one it will then be necessary to
complete the application and medical documentation
forms.
